Fallout 4’s aiming has always been perfectly stable. Whether you’re holding a lightweight pistol or a heavy sniper rifle, your weapon remains almost perfectly still as long as you keep aiming.
Steady Hands introduces a realistic weapon sway and aim fatigue system that makes every firearm feel unique.
Weapon stability now depends on your remaining Action Points, weapon weight, Strength, and sustained fire. Heavy weapons become increasingly difficult to keep on target, while lighter firearms remain easier to control.
Every weapon now has its own character.
Long aiming sessions become a tactical decision.
Controlled fire is rewarded over endless spraying.
Instead of adding random inaccuracy, Steady Hands simulates physical fatigue. As your Action Points become depleted, weapon sway gradually increases until you recover, creating a more immersive and believable shooting experience.

How it works
Holding your weapon consumes Action Points over time. Heavier weapons consume more, while Strength and Power Armor help reduce fatigue.
As long as enough Action Points remain, your aim stays steady. Once fatigue begins to build up, weapon sway gradually increases, encouraging shorter aiming periods and controlled bursts instead of constant precision.
The result is a combat system that feels more physical without becoming frustrating or unfair.
Compatibility
Steady Hands is compatible with virtually every weapon mod since it works entirely at runtime through F4SE.
No weapon patches are required, and both vanilla and modded firearms automatically benefit from the new aiming system.
Installation
Install using your preferred mod manager.
Configure the included INI to customize fatigue, weapon sway, and gameplay to your liking.
Uninstall
Uninstall at any time using your preferred mod manager.
